Athletic Physicals:
The Ridgewood High School Athletic Department has coordinated with Amita Health Resurrection Medical Center to provide an “Athletic Physical Exam Day” at the high school.  This is a one day opportunity for our Ridgewood High School student-athletes to get their athletic physicals completed in a convenient location by local medical professionals.  Details below:

• When:  Thursday, June 16, 2022
• Where:  Ridgewood High School Auxiliary/Old Gyms
• Times:  1:30-4:00pm
• Cost:  $25 (Cash or check made payable to Ridgewood High School)
• Appointments:  The physicals are by appointment only. There are a limited number of available appointments.  Please call or email Athletic Director, Rob St. John at 708-697-5540 (rstjohn@ridgenet.org)  to schedule your appointment.  
• Please complete and sign the first three pages of the IHSA Preparticipation Physical Evaluation form. Link to the IHSA Physical Evaluation Form.  Be sure to bring the entire form with you on June 16th - the doctors will complete and sign the last page of the packet. 
• Please be aware!  This event  is NOT meant for completion of a school physical. This is for IHSA Athletic Physicals only.
