School Health Requirements
Please note when submitting health records:
Physical exams and required immunizations are due by the first day of school. If not received by October 15, students will be excluded from school until the health requirement is met.
The Health History portion of the physical exam form must be completed and signed by a parent or guardian. Incomplete forms will be returned.
If your child did not receive a vaccine because they have had the disease (measles, mumps, chickenpox/varicella, or hepatitis B), the Alternate Proof of Immunity portion of the physical exam form must be completed by the health care provider.
If not up to date on minimum immunization requirements, a catch up immunization schedule must be provided from a health care provider in addition to proof of an upcoming appointment to receive the required immunizations.
If your child does not have or will not be receiving an immunization based on religious beliefs, a Certificate of Religious Exemption form must be completed by both the parent/guardian and health care provider.
If you’re unable to obtain a vision or dental exam due to certain circumstances please contact the nurse for information on obtaining a waiver.
Health Related Absences
If your child is absent related to illness or injury absence email the attendance office at Orysya Tsymbala and include the nurse office at Sarah Bazarek
